How to make a request
- Use the Information Regulator’s Form 2 — Request for Access to Record.
- Identify the record, the right you seek to exercise or protect, and why the record is needed. Include contact details and evidence of authority if acting for someone else.

Requests are considered under PAIA. Identity checks, lawful refusal grounds and prescribed fees may apply. A request does not automatically give access to another person’s private information.
